What can we do with our underclass? The only options I see are we keep paying them or we kill them off.

Most suicide methods have a significant failure rate. For someone who wants to die, there's nothing worse than surviving and spending the rest of the life as a vegetable.

Assisted suicide would be a virtually guaranteed successful death.
