Under what circumstances is it okay to leave a job without giving notice? (company, temp)

I know the old adage is never to do it because it burns bridges, but let's say your safety is at risk, or something else. What reasons are there that make it okay to leave on the spot?
you'd need to be more specific on your safety is at risk.. I would always say to give two weeks notice, if you can't stand the place then you could always use up your PTO before your effective date.
Failure to pay...
physical violence/threats
unsafe working conditions (Like someone welding near propane tanks and you inform the management of this and they do nothing)
i left once without even telling the boss, foreman crawled me for something i didnt do, so i just went home, never look back
