I thought the weather,food prices and many of the natives hate the whites, would be the most difficult challenge. I found the natives more welcoming than the workers and management at the hospital. The position I had, management had gone through 4 people in 2 years. That's not including the travelers they had there as well. If you really plan on going, don't bring your whole household. Bring what you will need for 4 months tops. Nursing has almost as much turn over as well. They are viscous and ruthless.
