Spend some time and research Mexico and Central America, particularly Costa Rica and Panama. There is a huge medical and dental industry there catering to U.S. citizens. There are many online forums and message boards that you can read, research, and post any questions or concerns you might have. They are overwhelmingly positive and glowing in their comments. When I researched this before, Cuba had not opened up yet but they would definitely be worth a look as they are known for their docs and training.
Many docs and surgeons in these countries were either schooled in the U.S. or at a top flight school in another country. Also many U.S. hospitals have branches and affiliates in these countries. Many of the medical providers also have liaisons that will assist you in every step of the way ... transportation there and while you're there, housing, meals, follow-up care, etc. Many even have onsite housing. People post that even with the expense of travel and staying there, the overall cost is still a fraction of what it would be in the states. Many also post that their insurance in the states even covered some of the medical/surgery because they realize they would save money by doing so.
Do your due diligence and research on this and you will quickly find that there are many, many, more citizens seeking medical help outside of the U.S. than there are people coming to the U.S. for care.
