I've seen them all across the West on steep declines. They are usually called runaway truck ramps.

Yes, there are several in the Appalachian mountains but they are different. Instead of going downhill into some dirt, they are super steep and uphill.

They are usually on very steep downgrades, like here on I-70 in Colorado

west of the Eisenhower Tunnel.

That's how I remember them. I always thought to myself going past one, that if I was driving a truck with failed brakes I might decide to take my chances continuing downhill rather than hit that thing. It looks brutal.
