Voodo doughnuts in Portland OR. and Top Pot Doughnuts in Seattle are the two top in the northwest.

NYC (naturally) has some great doughnut shops; the Doughnut Plant (Lower East Side) is Nirvana for doughnut lovers.
Probably the best we have to offer here in Atlanta is Sublime Doughnuts.

I would say LA. There are more privately owned doughnut shops there than any other city I have been to, by far.
