I'm a bit confused that only two Bay Area cities appear on this list and San Jose is that far down. Does it change a lot if it includes the 100 most?

These are metro areas, not cities. I emphasized that in the title and description. It wouldn't really change a lot. Santa Cruz MSA has 5 companies, and Napa, Visalia and Salinas each have 2. If you add these up and include San Francisco MSA and San Jose MSA then the entire area would have 196 companies on the list. That is still behind Chicago, Atlanta, LA, DC, and NYC.

To be honest, I expected more, but it is what it is. The most surprising to me in this is DC and Atlanta areas each with so many.

Ah, my fault, didn't really do the whole reading thing. Makes a lot more sense now though still surprised by San Jose.

San Jose is surprising that it is low, but to be honest - it is even low in the Fortune 500 rankings. Being the bay area though, I did expect more fast growing companies in San Jose though remember, these are 5000 companies that are fast growing of any type. The fastest growing company on the list is an LA-based company which sends people a box of items in the mail of apparel, gadgets, etc. The second fastest growing company in America with $2M+ revenue is a company that hosts group painting parties.

They have things by specific industry it's a little different maybe. If we only picked Software, IT Services, Telecommunications, Engineering, and Computer Hardware, then here's a look at some MSAs:

DC: 115
NYC: 103
Atlanta: 65
Los Angeles: 59
Chicago: 58
Boston: 39
San Francisco: 39
Dallas: 38
Philadelphia: 33
Minneapolis: 30
Seattle: 26
Miami: 23
Houston: 20
Portland: 18
Austin: 16
San Diego: 16
San Jose: 15

San Jose and San Francisco MSAs combined still interestingly below Chicago, Los Angeles, Atlanta, NYC, and DC.

It's remarkable how Ohio's "3-C" metros (Cincinnati, Cleveland and Columbus) placed so closely together in the middle of the list.

It probably has to do with the cost of the area. The entire Bay Area is very expensive, so it would make sense that the number of fast growing start ups wouldn't be as much as some other, less costly cities like Chicago, Philadelphia, Atlanta... even LA and DC.
