I've just started the South Beach diet and my doctor wants me to write down everything i eat for a few weeks,rather than scribble everything down on a notebook i am trying to find a down loadable daily food log that i could just fill in,but i'm having a heck of a time finding a suitable blank log page for the purpose, any one else have any ideas?

Check out fatsecret.com

They have a food database where you can enter what you ate and the quantity (say for instance, 4oz boneless skinless chicken breast) including a lot of brand-name items and it will enter all of the nutrition data into your food diary and add it up for you. You can even create whole meals or recipes as a single entry so you can enter the meal as one entry instead of having to enter all of the ingredients separately each time you eat it.

If you have an Ipod or Iphone, there is a free app called Fit Day. Its wonderful. Other than that, I write everything down in a spiral notebook. That way I cant lose any of it.

I really like Livestrong.com (the free version) you enter in what you eat and select the brands from the drop down menu, enter in how many servings and it calculates everything. You can download your weekly/monthly logs and print those to take to your appointments. It's an easy to use website.

I also recommend Fit Day.. If you are looking for a quick and easy way to just jot down what you eat...

Create an excel spreadsheet...
Get a journal

I currently use a journal so I can also record whats going on - did i meet my goals, not meet them, how come, etc... I also see a nutritionist once a week and I jot down my measurements as well..

My goal is body fat loss with lean muscle gain so I need to be on top of it ..

If you have an iPod, iPhone, etc you can download a free APP called "Lose It", you can set up your own plan, add / delete what foods you have eaten etc better yet the app has a feature where you can search foods either in the supermarket or restaurant and it has all the calories, sodium, carbs, etc.. VERY helpful. It taught me a LOT about restaurant foods and store bought foods.
