What I was told by a vet is that in some breeds, regardless if they're 'housetrained' or not still have a tendency to have accidents until a certain age.
Maybe that's what it is. Because he will go weeks sometimes and have no problems at all. It seems the times when the accidents happen are close together for several days and then he'll go back to staying clean and dry.
