You can do that in Austria and Germany. Pretty sure that that's standard in quite a few other countries. I'm not sure, though, which countries have the most efficient health care system.
In the UK, you would have to go through your NHS GP for a referral. Even if you have private insurance, you still need a referral.
Statistics show, amazingly, that the UK's monolithic state run NHS is actually one of the most efficient health systems in the world. Apparently, it spends a smaller proportion on administration than any other. Personally, I find it very hard to believe. It is the nearest thing to a state religion we have - any time its threatened with reform, the public goes mad. In truth, its decrepit, has a funding crisis, is run for the benefit of the consultants at the cost of everyone else, (some) hospitals are like something from the Third World, and the whole thing needs scrapping and replacing by a social insurance scheme with private providers as happens in most of the developed world.
P.S. No, we don't want the American system. Most Brits seems to think there are only two options. Ours and theirs.
On the one side there is a study done by the WHO, on the other side there is a study by a Washington-based foundation called "Commonwealth Fund".
It's upon you to decide which organization you deem more trustworthy. Frankly, I don't care. Pissing contests like that get tiresome. Most of the countries within the Top 50 provide similar service anyway.

I'm not defending the British NHS - far from it. I would scrap the damn thing and start again. Unfortunately, it's me and about 30 others who agree!
