Sad story. Reminds me of the woman in Mexico city that died in a similar manner trying to squat a lot of weight. Remember safety first. The safety bars are there for a reason and if lifting heaving utilize a rack with them. Spotters are good but at the end of they day they are humans and mistakes can happen.
