I know guys and boys can easily be mistaken for a girl especially if they have long hair but I can't say I've ever seen a girl / woman who I first thought was a guy except for particular extreme cases.
I would say it happens fairly frequently but I imagine it depends on where you live. There are a lot of butch lesbians around here. Some of them look like guys and I cannot tell until I am up close or they speak. Many times they will look like a really "pretty boy" sort of guy.
I don't consider this a negative. I am bisexual and I find this sort of thing attractive as well as "standard" representations of male/female... but yeah, it is not that uncommon around here, really.
One time at work, a gentleman with a shaved head came in and he was wearing a dark hoodie (hood was down) and baggie jeans. I was a receptionist at a doctor's office at the time, so I greeted him and I said, "Hello. Can I help you, Sir?" And he said, "Ms. Can I help you, Ms." I was so embarrassed I wanted to crawl in a hole and die.
I'm 6'1", so if people aren't paying attention they just assume the shadow I cast is from a man.
There were also times in college when it was dark, my hair was in a bun/baseball hat and I wore baggy clothes and people initially thought I was a man, but thankfully they quickly realized I was a chick.
I recently addressed a six foot tall woman as sir. Once I got close enough I realized she was a woman and quickly corrected myself but it was too late. I just played it off and kept the conversation going as if nothing had happened. She didnt make any comment. I don't know her background or her orientation but in my experience she looked to be a butch lesbian - sorry if butch is a derogatory term, but I wonder if it wasn't in some small way a compliment? I mean if you are going to go hard a certain direction to dress and not look like a traditional woman you can't be too too up tight if a guy calls you a guy, right?
Also I took the title to mean the OP was asking if women ever made this mistake abou other women.
My 20 year old brother has nearly the exact same body I did when I was his age, it's very odd. I went through very late puberty due to a genetic disorder I have so I did look like a boy (Plus, I was 6'1) until I was in my late teens.
Now, you'd have to be an idiot to think I was a man, but I have been asked if I was a transsexual or drag queen more than once. Apparently I look like a man trying to be a woman
I'm tall and for about 8 years I sported a very short hairstyle that could be worn by men. When I'm wearing feminine clothes, there's no confusion. But if I happen to be out and about in jeans and non-figure hugging clothing, and people are talking to me from behind and don't see my face, there is the occasional "sir" comment. People associate really short hair with guys. That's just how it is. When it first happened, I was offended. Then I realized that from a distance and from behind, people can't tell gender and go by such things as clothing and hair.
I've since grown my hair out some and I get no such comments any longer.
I made the brilliant decision to cut my 36" hair into a pixie cute in the middle of winter. Between black winter coats that could be considered unisex and not extremely feminine tobogans, everywhere I went people thought I was "pretty boy/Justin Beiber wannabe"
Yes it's happened to me a few times when my hair was really short. A man has never mistaken me for another man. It's was always a woman who got my sex wrong.

As with all things it depends on the person. My wife has been called "sir" and it doesn't bother her. Whatever pronoun you use, it's all good, as long as you aren't rude (like the time some old lady very rudely told her to get out of the women's bathroom, or when an office worker at our daughter's school referred to her as "he/she/it")... but I've known other lesbians who really do "butch it up" and then get mad when someone calls them sir. I never really got that...

I had a pixie cut, ONCE. Never again. I envy those who can pull it off. I cried when I looked in the mirror... I looked like my ex-husband. Don't ask me how that is possible but it was. I hid in the house as much as possible, then I dyed it bright pink and wore a lot of dresses to try and lessen the pain of the pixie.
The dumbest thing; I had just come out to my mother as bi (everyone else knew a decade beforehand) and I had just moved in with my then girlfriend (now wife). My mother thought the pixie was about me "Trying to look like a lesbian" and that my girlfriend had pushed me into doing it.
