The picture image looks familiar but I could just be remembering clip art images from forever ago. I would ask in your local dry cleaners if the have an onsite tailor or knew of one. If you don't want to silkscreen the material yourself look in the phone book for printing and embroidery shops. They do that kind of work. They also may know of someone who could complete the dress.
