Very rarely. I think the last time we went to a buffet was in 2011. I don't generally like them, in part because I only eat one plate of food, and the food selection is not usually what I'd choose to eat. I never cared too much for Chinese buffets, because nearly everything tastes candied. Savory is more my style.
I suppose this doesn't include all you can eat salad bar like Souplanation/Sweet Tomatoes where the calories you're tucking in from a mountain of spinach is pretty minuscule unless you douse the mountain with olive oil.
Nowadays, I visit a real buffet like maybe 1-3 times a year. I used to go to a Chinese buffet every week for the summer when I was out on audit with my former boss.
Any weight issues?
I don't even remember the last time I ate at a buffet. I don't eat much, so it simply isn't a worthwhile expenditure for me. I have taken my son, years ago...for him it was a bargain. But even he doesn't want to go to those places anymore. I mean, who needs "all you can eat" anyway?
Almost never. What kind of quality do you expect for all you can gorge on for less than $20? Are buffets even necessary? Americans are obese enough already.
1-4x monthly but I reside where the buffets 'live.' Thanks to genetics, no weight issues; to control portions, I use the salad or luncheon/sandwich plates vs dinner plates & allocate 1.5-2 hours since meals = social event [so we're eating slower which helps with better digestion].
I have no problem with buffets; I think it's very possible to go there and not gorge. Some people can handle it, some cannot. I go probably 2-3 times a year. When I am visiting family in Nevada, we always go to Toucan Charlie's in the Atlantis hotel. That is a great, upscale (not cheap) buffet. Locally near by my house there is a chinese buffet that is also very good. I go there maybe 1 or 2 times a year and tend to get the same things, but it's good and I like picking and choosing my meals, with just a tiny serving of each. My rule is no more than 2 plates. I usually do 1.5 I never fill up on the rice/potatoes/pasta/bread/sauces but tend to go for the meat and veggies.
The best buffet I've been too is at the Sanderlings restaurant at the Seascape Resort in Aptos, CA. Delicious upscale food...to die for! I love to go for their weekend brunch when we're staying at the resort.
I'd say about 1-3 times per year. I am not the type of person who eats for enjoyment. I eat to fuel my body, so I am not the type who posts pictures or recipies on facebook of food, like most of the "love to eat" types.
Last time at an all-you-can-eat place was probably early 2012 or maybe late 2011.
Chinese seafood at thezenbuffet.com and Japanese seafood at Todai restaurant in Woodland Hills and Studio City.

I had the Bellagio buffet, decent food considering it was such a wide spread from all different cuisines, all for $28 which was cheap for Australian standards. The range of desserts was nice too.
I generally go perhaps twice a year on average. We have a place called Sizzler where you get free 'salad bar' with a main meal.
Rarely, if ever, nowadays. Usually, it's been on vacation. And yeah, the cleanliness thing gets me a bit.
I've never had a problem with portion control there, as I'm pretty selective about what I eat. So it's not like I fill my plate up with lots of different things. It is nice, though, to be able to put together some pretty freaky combinations of what I do like, though, that you'd never find on a menu.
