It's not intended as a pre-emptive strike against receiving a gift. It's a pre-emptive strike against waste. I don't see how that's a bad thing, for the customer OR the restaurant.

lol - that sounds like me. I'll try just about anything, foodwise, once.

My comment would be.."Thank you, but no, I need to watch my calories! Maybe another time" Everyone will understand that.

my diet doesn't allow that.
my doctor said to stay away from it."

If you have to, open up the bag like you're checking to make sure it's all in there, and if you see any extras ask them to remove it (acknowledging the gesture, of course), and re-staple it. I hate for food to go to waste, so I'd speak up.

Where are all these Chinese restaurants that give free food? The ones around here don't do that. Dang.

So here's what you do say:

Last time you gave us an order of ________ as a free sample. I appreciate the effort and thank you for your generosity, but if you had planned to include a sample this time, we'd appreciate it if you did not. It's just too much food for us, and we don't want to waste it.

Haha, yeah, I can see that happening. I know asking them to take it out seems like a rude thing to do, but I wouldn't want to touch it so that they can offer it to the next person. Or I might even ask anyone in the restaurant if they'd like it. Not that I'd be in that position; I'm pretty sure we'd eat what's given to us.
