Or would you say they're all equally unhealthy?
I think you listed them correctly.

Depends. Is it brown rice in the Chinese dish? Then it would be the healthiest of the three, especially if it had some fresh chopped vegetables in it, not just a few frozen peas and frozen corn kernels.

They are about equally bad... refined starches combined with fats. Carbs plus fats in the same meal bring on insulin resistance over time. Brown rice doesn't add much fiber or nutritional value. Eat all these in moderation, not part of the daily diet.

Eat a higher carb, lower fat diet, or a lower carb, higher fat diet. Don't eat a high fat, high carb diet. If I'm going to splurge on a high fat, high carb item, I want it to be a quality item, so I'd go with the pizza from the Italian restaurant as my choice. I don't want to consume low quality, substandard tasting food, period. That's McDonalds in a nutshell.

Depending on the ingredients, the chicken fried rice could be slightly to significantly more healthy, but I still won't make it my daily driver. I would rather have a chicken and broccoli item with a small amount of rice on the side.

Define "healthy?"

If you were to eat nothing but one of those choices for an extended period, say, 6 months, then #3 would delay the development of deficiency diseases the longeest-- Chicken has a good mix of vits, mins & protein, while potatoes are the second best vegetable source of vits & mins (behind beans) and a good source of calories. #3 also has the best balance of protein to carbs ratio.

#1 at least has the chicken, but rice is a pretty lousy source of anything else except carbs, and #2 at least has the dairy to provide better nutrients than the rice.

I find that generally Chinese food is healthier, because their dishes are often a lot of vegetables with a small amount of meat for the flavor. With chicken being a healthier meat, the fried rice is best, followed by pizza, then McD is last. Any health benefit from the chicken rather than a hamburger is eliminated by the fries. Nonetheless, none of these is terrible for you, if eaten in moderation.
