I'm 38m

My Total Cholesterol is 256
Trigs 187
HDL 43
LDL 179
Chol/HDLC 6.0

I admit, I don't exercise enough, only somewhat watch my diet and have a desk job, slightly overweight.

Can this be changed by diet/exercise and fiber changes or is this urgent and in need of meds regardless?

Exercise will increase HDL. Diet + exercise will lower LDL.

I am very skeptical about the medical profession stigmatizing cholesterol. I am very skeptical that they have proven a causal relationship between high cholesterol and the risk of dying prematurely from a heart attack.

I am no doctor and I have no medical advice but if you want to try to lower your bad chloresterol, trying eating more eggs, then your liver won't have to produces as much LDL.

Dietary cholesterol may in fact be good for you. Your brain needs it. Your heart needs it.

Recent studies actually show the opposite -- that higher cholesterol actually is associated with lower all-cause mortality, and better cognitive function in your later years. High cholesterol runs in my family and I know several family members who have been on meds for it for years on end without any results. They exercise and watch what they eat and its just something that sticks with them regardless.

High triglycerides usually result from metabolic syndrome, which results from eating refined carbohydrates along with no physical exercise.

High triglycerides results in high total cholesterol. MDs assume that results from diets high in cholesterol and/or natural fat. NO.

High cholesterol results from the body making extra cholesterol to repair damage from chronic inflammation, which results from metabolic syndrome.

Metabolic syndrome leads to type 2 diabetes and cardiovascular disease.

MDs have basically all the wrong ideas about cholesterol. In only a minority of cases, extremely high cholesterol results from genetics, and those people need the drugs. No one else benefits from the drugs.

Your MD will tell you, however, that it's genetic and will try to sell you on the drugs. If they mention diet and exercise, it will be with the understanding that you won't do it. And taking the drugs is easier anyway.

The numbers aren't the whole story, ask for an NMR test, if your LDL is the large, lofty particles it doesn't stick in your arteries. The HDL should be small particles to keep the LDL out. The doctors won't tell you this or order an NMR test for you, they want to sell you Lipitor.

The science behind LDL sizes is still in its infancy. I would say if his cholesterol is trending up this is a sign he’s gradually becoming less healthy whether cholesterol is correlative or causative. Instead he should exercise and diet and I bet his cholesterol comes down.

Infancy? I had the particle test done 20 years ago when my primary physician wanted to give me Lipitor just looking at the numbers, the test was called something else back then. After the doc looked at the results he agreed with me, I didn't need the pills and my count was 280 with large particle LDL and small particle HDL, like it should be. I read about the particle test years ago on Dr. Mercola's web-site.
