Most servers are running firewall block lists that ban IP's. These lists are downloaded from external sources, usually once a day. For example if I want to block all of China I can do that, In my case I'm more interested in ones that target forum spammers. That site is making requests to the host as a proxy, if enough people start using it then it's going to come under scrutiny and could/will up on a block list if it isn't already. In addition to that many webmaster/server admins monitor their sites for such traffic automatically and it could end up on the other side of a firewall individually.
