I applied to a handful of Goodwill locations, received and went to the interview at one location, then received a phone call from the manager who asked me to put in the Requisition Number, fill out the info, and he would get back to me. I did what he asked, but have not heard back, so did I do the right thing or the wrong thing? He had told me on the phone, to enter the number, fill out the info, and he would call me back to come in and fill out the paperwork. I heard from him this past Monday.
I don't want to call back and feel like an idiot because I didn't do it the right way. :/
If you did fill it out incorrectly, better to call and follow up and find out if you need to do something differently than miss out on a job and wonder forever. Waiting is the worst. Following up shows initiative and desire for the job.
How are a bunch of random people on the internet going to be able to answer your question? None of the random internet people saw whatever it was you submitted, so no one has any way of knowing whether it was right or wrong. You will have to call the person who received the information to get your answer.

Looks like there is a number, probably in an email or on one of your hiring forms, that identifies you are a candidate for employment. It would have been issued by the hiring company to you. If you can't find it, ask them and I'm sure they will provide it to you.
Maybe you didn't do it right away, but the longer you wait, the worse you'll look. Do it right now. If you can't find it, give them a call monday morning, or tomorrow morning if they are open on sundays. Or, send them a quick email.
Don't defend yourself or apologize (unless an apology is warranted). Just ask for the requitition number, say you aren't sure what it is and they will give it to you.
The simple fact that it has taken you this long to pick up the phone proves they were right to pass you by.
