Over 3 weeks ago the loan processor contacted us, we paid for appraisal, we were told we would hear back in a couple weeks. Appraisal was first done a few days ago. Our loan commitment and appraisal contingincy is up on Monday with no chance of extending. We were asked for one explanation on something very minor in the contract last week (from underwriting?) is this normal timelines or should we be concerned?

I can't speak on specifics for your case; but can say the real estate market here is crazy busy right now. Has been for awhile. I would suggest contacting your lender with your concerns as (assuming there are no issues in play) they should know what the real world timeline is right now and be able to work thru that with you.

There's a ton of very last minute stuff involved in such real estate deals. It's just part of the process with so many simultaneous things having to be worked out and timed properly.

Why aren't you speaking with your attorney regarding this? I'm in the middle of it now myself, and the attorney is sorting out most of the 'crud' for us.
