In Queens - Trattoria L'Incontro in Astoria is excellent. Ottomanelli's in East Harlem is supposed to be very good.
I have heard CONSISTENTLY the best Italian food is on Bedford Ave in the Bronx. Little Italy still has good restaurants but it is no longer the premiere spot for Italian.
Go to Forlini's on Baxter st 1/2 block south of Canal, right by that Chinatown tourist information pagoda. If you want an oldschool place that has been there since it was a working class neighborhood (40s) and don't want to spend 200+ for dinner. The portions are huge homestyle portions. You can get two apps, two entrees, bottle of wine for less than 200.
Got this tip from a guy who used to work in restaurants in the city and I was not disappointed. Best Italian food I've had besides my grandmother or one of my friends mothers.
