I was thinking diabetes. Excessive thirst, itchy skin, frequent urination are all symptoms. I am diabetic and I am always looking for these symptoms in my own children.
If this is relatively new, I would have your pediatrician check for a UTI before anything else.
Is there anything that is causing stress? Note that potty training is one thing that only the child controls. If she is not allowed to control other things, she may be doing this to show you she has control.
No juice, no Kool Aid, nothing but a little bit of milk, and water when she is thristy. Let alone, no soda. That will probably solve the problem right there. I made that rule for my little ones, and let the older ones drink their yummy juice boxes right in front of the little ones. And it was, "Oh, yes, I know you want an Apple-Cherry-Blue Raspberry Juice box..., but those are for BIG kids, who don't have to change their pants"...and of course, there was screaming, and crying, and carrying on...and then, a "miracle" no more potty problems....and then, everyone could have a juice box.

Also, I am surprised no one has mentioned it yet, or maybe I missed it, but get rid of the Pull Ups! They are confusing for a kid since it says "go ahead feel free to pee in your pants, even though you are almost 4"
Those are the worst invention ever.
She sounds JUST like my daughter, very strong willed, and laughs. She doesn't care when she is in trouble.
I went through this however with my son. NOTHING worked. When he was ready, he stopped having accidents. I would suggest if you want to try something, do it positive.
Create a chart and every day she is dry for the entire day she gets a star. If she does it for a few weeks straight she can pick out a small prize. Sometimes, the stars are enough.
I think it's something that will certainly fix itself with time..A lot of young children will laugh when they've done something wrong, to them it's definately better than crying,,Be patient, I've known people with kids that pee their pants a lot, but as soon as kindergarten starts, miraculously they stop..
She ONLY wears pull-ups in bed. She puts them on right before getting into bed and takes it off and changes into underwear before coming out of her bedroom in the morning.
I don't personally believe in giving toddlers, kids or my 30-year old son money for accomplishing something. I'm the mean mom who didn't hand out five dollar bills for good grades. Can you tell?

Incentive TOTALLY works. I just don't like the idea of teaching a 3 year old that she gets cold, hard cash for a reward. Give it to a three year old and she's going to have her hand out when she's 15. Put me in the give-them-stickers category.
