Trying to figure out what is wrong with our tree. We've had it for over 12 years and the leaves have never looked like this. Any ideas?
I see no bug infestation.

How are you caring for it, like watering and fertilizing.

we lost 1000's of Ash trees here in Illinois in the last couple of years due to the Emerald Ash Borer...could it be that??

We don't have Emerald Ash Borer in Arizona. The furthest west it has made it is Colorado according to the USDA map.

I'm not fertilizing it at all. I water the tree once a week.

Though we obviously aren't in Yavapai county, it sounds like the symptoms might be similiar to what you're experiencing and it might be AD:

That size tree should only be watered once every 4-6 weeks deeply during the warm/hot periods. Weekly shallow watering is not good for the trees. Fertilizing with a general purpose tree fertilizer in the Spring helps with growth and a healthy strong tree. Follow up with a deep slow watering. Weak trees are subject to several problems especially decline.
