Never had anything like this happen before. Don't pay cash! Do I ask them for Doctor I.D. #'s or something and complain with the state? It's a small office in Tempe, not sure if it's doctor owned or what.

Don't pay cash!

I have a printed receipt, I think the underlings (more than one) pocketed my cash...

After calling the state and them to see what was going on, imagine this, my paperwork gets submitted!!!!!!!!!!!!!!!

Not to be offensive, but you sound like you need an attorney do this for you.

Usually these matters are done on a contingency (Lawyers make money off the verdict/settlement).

Good Luck.

State Bar of Arizona :: Protecting the Public While Advancing the Legal Profession
(602) 257-4434

Get your stuff together, and calmly call the number in the link and explain whats going on and they can point you in the right direction. A rule of thumb is to avoid those lawyers who advertise on billboards or television (because they work on volume and not for individuals).

Don't use Avvo, it's just a pay-marketing, platform. Any Lawyer you are referred to, before you go for a consultation check their record here: State Bar of Arizona :: Find a Lawyer to make sure they haven't been slapped for violating ethical rules.

When you find them in the State Bar Directory, there will be markings if they have had some malpractice claim brought against them successfully, don't hire those lawyers for obvious (hopefully) reasons.

You should be able to get an inexpensive consultation with an attorney who has experience in the medical malpractice field. I don't have any idea what you mean by "don't pay cash" and "doctor's ID number", "submitting paperwork" and "pocketing cash" etc. Sounds like your complaint is more with the office than the doctor... and that's not malpractice.

Depending on just what the nature of your complaint is, you can also file a complaint with the Arizona Medical Board. You can find the information on how to do that on their website.
