Here's a link to the template letter. It's got an obscure name "HOW TO FIND OUT HOW MUCH THE OTHER PARTY IS PAID AT
WORK" . BenBermuda, I assume you have filed court papers to open a case #? You need that to make this letter valid.

What the OP is doing is a MODIFICATION of child support. So, I suspect there is already a case number from the original decree/orders.

It's a letter template you send to the employer. I found it under the child support section of self help. I think they call is "how to find out how much your ex makes" or something along that line. I did find subpoenas for other states that you can use to obtain pay check information from employers. I am new to Arizona and still learning.

My ex's attorney was able to get copies of my Wells Fargo bank statements. Wells Fargo has a subpoena department. The reason I know they obtained copies of my statements is #1 Wells Fargo sent me a letter letting me know they requested them and WF was sending them out and #2 they had copies of the statements in the hearing.

The purpose of getting bank statements would be to find out how much is being deposited each month and compare to the pay check information. The ex may have a 2nd job I am unaware of. I have to dig out the letter WF sent me in order to contact them. I did a search online, but couldn't find that department of WF.

Call WF and ask to see the documents. If you can't find the letter, just call their main number and ask for the right dept. You should have a right to see the subpoena they got, and to get a copy of it. What's good for the gander is good for the goose, to paraphrase an old saying. Make one up just like it, for the ex's bank, and submit it to the court to be issued. There might be a copy of the one your ex got in the court file, but not necessarily.
