I would have to give it to Eleanor Roosevelt as she used her "position" for lots of things beyond the ceremonial. The same could be said for Edith Wilson, although it was not as well known.
Hillary decimated women (either career women who took time off or regular homemakers) for choosing to stay home and raise their children. What a nasty schrew.

You think so, but I think otherwise. Hillary was an example to little girls that females could aspire to what they wanted to do, much like Eleanor had been. As a child of the 50s, I saw Eleanor as the only example of that.
Although I STRONGLY disagree with her politics, and found " the war on drugs" to be horrible for our country, I admire Nancy Reagan for her fierce loyalty to her husband and Margaret Thatcher style toughness. She was running the show for him the last couple years of his presidency. Getting shot did that tough old guy no favors.
In some ways, she was also the worst First Lady. But she sure made an impact.
