Reporters on a regular basis twist stories for rating purposes. Most of the time the only people that know the story wasn't totally accurate are witnesses or people who know those who were involved. Knowing is not true doesn't really matter if you don't have the power to be heard. Responsibility is debatable. Humans have a great capacity to lie to themselves and justify those actions that don't pass the self deception test. A reported would say to himself "I need to feed my family" "misrepresenting this event is not gonna cause any damage" or "well people shouldn't be that gullible and believe everything they see on TV"

The people coming up with the theories suffer from the same things as reporters

The people teaching the false theories however usually are not even aware that their sources are incorrect. Humans don't have the time, experience or resources to prove every theory that is presented to them.
