Used to be a caregiver that would bring an old couple's car into the shop for service, naturally they would be tagging along. I hated working on that car as it would wreak of urine & baby powder.

I will say, it is understandable if they have no control. I don't hold that against anyone. However, to willingly put on a diaper for role play and do your business in it is vomit worthy!
