I typically ask the lady what she would like and then I place the order. (I have a few languages and my lady friends don't always have English, however I still feel it is classy for the gentleman to ask the lady what she desires, then I order starting off with...'The lady would like to have'...)

I appreciate old fashioned chivalrous gestures, but this isn't one of them. What if the waitress asks if I want a modification or temperature? I have a mouth and I like speaking for myself.
Having the guy let me order first is chivalrous enough for me. I'm picky...I always modify my dinner.
