For 30 years of my adult life I have been single and unattached (including now) and I have never been asked out on a date.

Need to add that I have spent much of the last 22 years being gainfully employed and working in highrise office buildings. 14 of those years single.

I have seen many women approach certain guys and pursue them, and even ask them out. When a woman sees a guy she is really interested in, most of the time they have no problem being the pursuer. I have seen it too many times.

In the beginning, it's (generally) up to the men to initiate. Some men, particularly those that women consider very good looking, will have women initiate with them but that isn't the norm.
