Yep, I think the therapist is the place to start. By the way, OP, I think its very brave of you to post a thread like this and talk about it openly. It's a good sign.
But I really hope you do some honest self assessing and ask yourself why you want to this. If you decide it's so that you can enjoy sex and have a normal sexual relationship, you should go forward.

Not only the sex issue, but it would be nice if I could tolerate a gyn exam. I'm not worried about cervical cancer, but I have had some menstrual issues that need to be checked out.

Oh, I didn't take offense. He's actually right. I do need to learn to relax. It just might take professional intervention to help me learn to relax.
I think that's way too much money personally for sex and genophobia. I agree someone else that said it's an irrational fear, that's what all phobias are though. But it's your money.
Well, I think I need a hymenectomy... but my old doctor couldn't tell me about the procedure and whether or not insurance would cover it. I'm reading it's considered cosmetic and wouldn't be covered. I am asexual, but I'm thinking when I move I want to ask a new doctor her opinion. Exams are VERY painful for me... so if the new doctor is straight forward, I might see if I can make an exception since I would need this surgery for exams (as it stands now, they can't do full exams because they can't get through).

But, I'm not sure if it's vaginismus, like VanillaChocolate was talking about. I've read that those two conditions can be hand in hand... you can get the hymenectomy and still can't be penetrated because of the fear, which then it'll take the dilators and therapy. I don't trust my old doctors, so when I move I will definitely be asking. Also, when I move, I will be living by myself so if need be... I could use the dilators.
