Say you're on a date with somebody you've recently met. You don't know much about this person, not much about their religion, etc. You're at a restaurant with them, waiting for your food, and you notice they make the sign of the cross, briefly pray (not out loud), and then make another sign of the cross.
That would be totally fine by me. I would probably ask them about it and see where the conversation leads. I would probably take note of them being dedicated enough to take a moment to pray.
Given that I am not very religious and could not envision being in a relationship a very devout person, I would think that this will be a our first and last date.
are they saying grace before the meal, or just doing this just to do it? I would ask them about it, out of curiosity. I'm sure they would know that this is an action that would provoke a question?

Yeah I agree with that. They would surely know that, so I'd ask.
When I saw the thread title, I thought it was about someone wearing a cross on a chain. Obviously I am not catholic!
By the way, my son has gone to catholic church with his dad, for years. He probably wouldn't say anything.

I'd politely ask, too. I have no problems with saying grace before a meal, but I would not want to date a devout Catholic. I'm Episcopalian, and we make the sign of the cross during some prayers, so I wouldn't automatically assume the person's religion.
My ex in laws were catholic. They did this before meal.
