For me (on a woman). It's the bad teeth. I've waited on enough meth addicts at the pharmacy to realize just how horrible missing teeth look.

arm pit hair that is so long they have to tuck it in...you are allowed to trim it to a respectable length.

mullet, bad teeth, overly coiffed hair, wears more than a wedding band, watch and class ring, poor hygiene, piercings...the list goes on and on...
