We belonged to both and they were both less than a mile from our house.
Costco was bigger and had more stuff, but we never went there because it was always too crowded and the checkout lines were too long.
Then we moved.
Now Costco is 70 miles away and Sam's is about 150 miles.
We don't go to either one.
When I read Consumer Reports monthly, I note the products rated highly that are sold at Costco. Most are even CR's "quick picks". I've not seen the same reviews for anything from Sam's nor BJs.
To get the prescriptions you dont need a membership. If they service the walk in public then they are legally obligated to service you too! (with or without membership)
In San Diego, there is only one Sam's Club, and numerous Costco's. I don't know how Sam's Club stays in business, but they do. But I'm a die hard Costco fan, and got a great deal on my new Michelin tires there.
But here's an example between the two stores: My ex-boyfriend bought a battery for his van from Sam's Club because it cost less than Costco. It kept going dead on him, and they kept replacing it. After the third time it died, he got his money back and bought one at Costco. And it never had a problem, either.
We once joined Sam's Club as part of a mystery shop that I had, but I got to keep the 1 year membership. We didn't renew. It was just bigger amounts of the same stuff they carry in Walmart (or .com), with the same pricing.
We recently moved to a rural area, and wanted a place that we could buy in bulk, to cut down on trips to "town". We joined Costco because their prices are lower in bulk than Walmart and I wanted some items friends had that I couldn't find anywhere else (a large storage bench for pool toys and a really cool garage organizer). The fact that their prices are ACTUALLY cheaper is a bonus.

That depends on the state. It is the case in Illinois, but not in all states.
