I spent five years in Moscow as a correspondent in the 1980s. For one flight back to the U.S. I got an incredible business class deal on SAS through the American Express. When I actually got the ticket, it turned out to have been issued in Karachi for a flight from Karachi to New York via Moscow. I just ripped off the Karachi-Moscow ticket and had no problem at the airport. In those days you could just join the itinerary in mid-air, so to speak.
