Do the HOV lanes make much of a difference with commuting times?

Yes and no. Depends on where you are traveling.

My fiance and I work across the street from each other in Bethesda and HOV from Gaithersburg most days. It cuts about 15 minutes off the commute. Without HOV it is a 45-60 minute drive, but when we HOV it can take as little as 30 minutes. It's so nice.

Too many cheaters in the HOV lanes mean that it's not that much different than driving in the regular lanes. It's a little bit better, but not much better.

No, it's really a lot better. The problem comes when cops park in the left shoulder to try to catch the cheaters. It slows the lane down a LOT.

I can tell you about "from exit 11 to exit 18 northbound" and yes there are really a lot of cheaters.

Actually it's A LOT better (at least on the I-270 corridor). HOV cheaters or not, there's no way you can compare the two during rush hour. I usually commute with a co-worker/friend but on days that I have to drive in alone, it's pure misery.

Okay. It's only marginally better when I'm using it. Must be that the cheaters all use it at that time and not at other times.
