As between Spain and Italy it's a tough call, but I would rank as follows:
Large cities (500,000+ city limits): Spain by a large margin.
Madrid/Barcelona/Seville/Valencia > Rome/Milan/Naples/Turin
Midsize cities (200,000 - 500,000): Edge to Italy. Spain has some outstanding cities in this category as well, but this is where Italy really shines and where it has its best cities -- some of the most incredible cities in the world.
Venice/Florence/Bologna/Verona > Bilbao/Granada/Córdoba/Oviedo
Small-ish cities (50,000 - 200,000): too close to call. I would call it a tie or a tiny edge to Spain.
San Sebastián/Cádiz/Salamanca/Toledo/Segovia = Parma/Modena/Bergamo/Perugia/Siena

Italy and Spain are the 2 most romantic countries in the World where it is easier

I recommend you a trip through Italy and Spain by strolling during the day
in different cities and eating in selected places and enjoying some summer evenings outdoors in wonderful music festivals that always is it so in places beautiful as the Pesaro
city of Rosini ,The of 2 Worlds in Spoleto,the one of Ravello,The Lucca Festivals of Puchini etc etc etc,.
In Spain in the summer in any small town go to the Castle or the Cathedral it is almost sure to enjoy the best music or also other days feel sunset on a terrace
best over the Sea with a sherry and exquisite fish and almost sure to meet people interesting.
The sumum would be in Granada in a Carmen or Similar with views to the Alhambra where by the way there is a exceptional Festivals of Classical Music in June in the Courtyard of the Palace of emperor Carlos V in the Alhambra.-
A trip like in this is a experience as a advance of the Paradise.
Unless that you travel as a suitcase and with the speed of a business trip as do many tourists and spoil a trip that could be wonderful through Spain and Italy and a wonderful memory forever-.
