COS- removed all potentially hazardous foods (PHF) and bagged up to be returned for vendor credit (all PHFs were put back for vendor credit). KDA_4_60111C - --There is dust and debris on the bottom shelf of the ice freezer of the retail area. There is debris on the floor of the WIC. There is an accumulation of cappuccino powder inside the cappuccino machine. KDA_4_60211E4 - --There is mold under the soda nozzles and in the ice chute of the Pepsi soda fountain in the customer self serve area. There is mold in the ice chute of the Coca-Cola soda fountain in the customer self serve area. There is mold on the inside rim of the ice machine in the storage area. KDA_6_50112A - --There is coffee debris and food debris inside the first and second cabinets from the wall under the counter with the coffee machines. KDA_7_20111A - --Located in the retail area, there was a container of hand soap stored next to two bottles of Heinz 57 sauce. COS- moved. Located in the storage room, there were four containers of hand soap stored next to a box of 20 ounce single use foam cups. COS- moved. Located in the storage area, there was a package of chlorine wipes stored next to a box of chips. COS- moved. KDA_KSA65_657b - --Located in the retail area, there was a box of 4 single dose packages of No Doz Alertness Aid pills with an expiration date of 11/14, a box of 20 single dose packages of Alka Seltzer Plus with an expiration date of 5/15, a box of 11 single dose packages of Pepto Bismol with an expiration date of 2/15, a box of 8 single dose packages of Acetaminophen with an expiration date of 11/14, one box of Motrin PM with an expiration date of 4/15, and five boxes of Pepto Bismol with expiration dates of 4/15, 6/15, 7/15, 7/15, and 5/15.
