On Volvos it's typically not the belt, it's the hydraulic tensioner or rollers. Belts do break but not as frequently as you'd think. I know there is an aftermarket silicone belt for my car that is supposedly good for quite a while.

On my 5 cylinder inline, belts are typically replaced every 70k and every 140k the tensioner, waterpump, and rollers are replaced.

My wife's VW has a chain, and it's very noisy. There is rubbing on the guides. It's fairly expensive to have someone replace it. If we buy her another car I'm thinking about opening it up myself because we'll have the downtime on the car.
