I would like to know if there is a full-size sedan that is available that is as quiet (in terms of cabin noise) and smooth as a Lexus ES (or close to) but available with manual transmission. A crossover (small SUV) with similar features is also fine.
I looked at Buick Verano but it is too small for us. Buick Encore, not so quiet, I think and not a good vehicle overall.
The Scion xD is marketed as a small SUV in other parts of the world as a Toyota, however those versions have 4WD. The US-bound xD is 2WD, but then again, many CUV's are as well. The ride is not smooth, and it offers no comparable luxury features you so desire that would come with a full-size car; but it does come in a manual. At least it satisfied one criterion.

That's tough, especially not knowing a budget.
Larger Sedans:
Cadillac CTS
BMW 5-series (but not on the base engine)
Honda Accord (though you can only get the manual on the I4 in the sedan)
Buick Regal (not as large as the others, but bigger than the Verano)
SUV's...
Porsche Cayenne (largest you can get with a manual)
Hyundai Tucson / Kia Sportage
VW Tiguan
Mazda CX-5
Subaru Outback
Subaru Forester
Nissan Juke
With the exception of the Juke and Cayenne, the manual is usually only available on the base trim and/or base engine on the rest.
If going the car route (not knowing budget) I would give serious consideration to the Accord. It is a fullsize car by most standards and while you are limited to only having a manual with the I4 in the sedan (the V6 coupe can get a manual) it's still a great combo.
On the SUV side, the Cayenne is the only larger one, but that's a pricey vehicle. The Outback and Tiguan would arguably be the best riding of the remaining options. There are other SUV's out there, but they are far more "truck" and wouldn't have the ride you are looking for.
