I'm sorry. I don't know what a post-secondary transition program is but for some reason this popped into my head.
Welcome to The Bridge School
I've heard that it is an amazing school though it may not be relevant to you.
But maybe it would be a good resource.

Default Transition Partnership Program

or TPP exists in our area but the student would have to be enrolled while he/she is still in High School. I believe they can stay in the program untill they're 22. I wouldn't recommend OUR area for anyone with special-needs kids, very few services and poor job market which means the work-experience agencies are competing with the rest of the population. My cousin in San Jose has had great services for her kid and Sacramento seems to have lots of services. Best of luck to you. Just had another thought- is your son a Regional Center client? The Regional Centers would have lots of info on services in their areas.
