What is forgiving? What are its elements? (church, Messiah, prayers)

Most of my life, I have struggled with forgiving. Forgiving Mother. Forgiving Father. Forgiving the abusers in the old church. Forgiving the old church.
Forgiving some of these has been easier than others, but my mind has been in occasional turmoil, wondering if I have forgiving enough, if I have forgiven at all.
Messiah said to pray similarly to this: "And forgive us our debts, as we also have forgiven our debtors." It advises us that when we stand to pray the prayers, if we have anything against another, forgive, because if we don't, G-d will not forgive us.
Regardless, when the act has (acts have) been heinous, life-changing, and are part of the memory, what does forgiveness toward those persons look like? What are the thoughts and actions of the one who needs to forgive? What if the perpetrator is dead or otherwise out of reach?
