If you are talking about Ph.D. programs- most people who stay in the program will receive A or mostly A grades. Advisor and referee evaluations of research will have a considerable impact in distinguishing between graduates.

A B in your PhD program meant a significant deficiency? In mine Bs were very normal. Most of my professors said if you were getting As in all of your classes you weren't spending enough time doing research.
A final GPA of between a 3.5-3.75 is very common in PhD programs and not once has anyone asked me what my actual GPA in grad school was.
So it matters only in that you need to maintain a B average to graduate.
