People lean toward using first names whenever that name is unique enough to do so.

George Bush used to refer to Saddam Hussein as "Saddam". NBA announcers say "Kobe" and "Lebron" instead of Bryant and James. If any of those names was "John" then we'd only hear their last names. If Hillary's name was Heather then she would be known as Clinton across the board.

HC's own campaign is employing Hillary on placards. For obvious reasons, she's not using only her surname.

Very true. What her campaign (and therefore it is her) demands is that only some people must use her full name or title.

The idea that using her first is sexist came from her campaign I believe. I guess as long as you support her, calling her Hillary is ok.
