That is very sad. I can say as a mother of three that if little babies were prone to dying every time their mother falls asleep while feeding them, no one would survive to grow up. This is an anomaly, and there may be some factors which we don't know about.
At the same time, some avid co-sleeping advocates claim that a mother is always subconsciously aware of where her baby is, even when she's asleep, and would never, for instance, roll over on him or accidentally smother him. That isn't true.
Even if suffocation isn't involved, there doesn't seem to be any 100% foolproof way of avoiding crib death. It happens sometimes. Very sad and tragic for this family.

If we can't prevent something 100% of the time then we shouldn't even try and no one's to blame.

What?! I didn't say that. Of course there are steps to be taken which can reduce the risk of crib death / SIDS. The fact that none of them completely eliminates the possibility doesn't mean they shouldn't be taken. And we don't know any details about this particular incident. There may have been risk factors and there may not have been. Suppose the mother was sitting in a recliner nursing the baby and dozed off. Is there any parent among us who hasn't done that? Would you really blame the mother in that case? Did you ever have a baby?
I fell asleep with one of mine while breast-feeding ..... I woke up around 2 hours later with him asleep and his brother crying to be fed.
Not really sure what happened here .... could be SIDS.
