I have been a Texan my whole life, aside from 2 years in Cali and one year in Oregon. As a kid, following what I heard around me, I used to say y'all ...and then my dad moved to Denver. Once I learned that not only is y'all not a word, it is also not an expression used in most of the 50 states, let alone the world, I made the concious effort at about age 9 or 10 to stop using it. It will still slip in every once in a while, as I am sure a couple of other "southern-isms" do in my regular conversation.
Here are some you will hear that REALLY bug me:

Buggy- a shopping cart
Feeder- the freeway access road/ frontage road
Fixin' (to)- about to/ will be very soon
Coke- any and every brnad and type of carbonated and flavored drink out there, whether it is actually Coke or not

Some of the little sayings are cute, but most of them involve things that would get me in trouble on the forum.

As far as m'am and sir...those ARE regular words, as in part of the English language for centuries. I seriously doubt that Texas/the South is the only place they are used....?

Y'all is just a contraction of you and all, you know that. Same as when I was in Chicago, y'guys was a contraction of you guys.

I use it everyday. Probably not Ya'll. Coz i am not a native southern.

But sometimes that Sir thing bothers me. I see man much older than me calling me Sir... and I think what did i ever do to be called Sir?? But that's how nice people are here.

If used, how common are they?
I wouldn't get caught dead saying y'all, but back when I dealt with customers for a living, I'd use sir and ma'am.

A lot of times it makes me feel old, but I have to remember that when I hear it, it is not about me, but rather it's about them. It's like when I hold the door open for a woman to enter someplace, it is not about her, she might even complain. It's then that I would like to tell her, "Sweetie, it's not about you, it is all about me."

I say Ma'am and Sir to my minor children. Just manners, not age. Children learn by example more than by speeches.
