From the looks of your fees (free exam/x-rays and $30 fillings) and treatment plan (desensitizing medication for every filling), I am willing to bet you have an HMO/DHMO insurance plan. There's your answer right there. Most dentists who accept HMO plans present extensive treatment plans because the insurance fees are so low that they need to make up for it in volume.
Seek a 2nd (and 3rd) opinion from a dentist.
