I noticed that at the end of the debate last night too! It's a coincidence but I've a relative who does this same sort of thing (will remain nameless) rather frequently.

It disgusts me (I certainly don't want to see another person's tongue), but mainly it bothers me because both McCain and this other person are rather mean-spirited people with sharp tongues. From a Behavioral/Psychology standpoint sticking out a tongue can mean many things but primarily it is supposed to be "silly" or "playful." Because the two older adults I know who do this are both rather mean people overall, I liken it more to being like a snake or having a sharp tongue. It's thus a warning sign to me when I see it. I saw McCain do it after spending nearly an entire 90 minutes reiterating negative point after negative point already addressed and counterpointed in previous debates about Obama. Now I definitely don't trust McCain. That's just my two cents.

Better to have lost control over his tongue than his bowels (which I am sure is right around the corner)
