Yeah, there are a lot of small towns, especially in the Mountain West, where people have a ton of money and care about their appearance. And even in small, poor towns in the Southeast, you'll see a lot of beauty queens. A lot of southern girls that go on to pageants came from small, podunk towns.
In small towns near the Rust Belt cities or western Pennsylvania, yeah I can definitely see why people would let themselves go. Those regions have a very rough and nitty gritty reputation, so the people tend to live up to it in their appearances as well. Almost as if they intentionally don't want to be attractive. They all want to look rough and intimidating. You see it a lot in small towns throughout Illinois, Indiana, etc.
