Is it permissible to live in one state and keep your medicare and supplemental insurance in a neighboring state? As an example live in Mesquite NV and use St George UT hospitals primary doctors and specialists. How about the same scenario only with a purchased (non Medicare) insurance for a spouse under 65.

Medicare supplemental insurance can be used it with any Medicare provider in any state.

Medicare Advantage plans are specific to the region where they are purchased. Some provide for out-of-network coverage but the plan pays less, sometimes substantially so.

Similar to Advantage, non-Medicare insurance as a rule may or not provide for out-of-network coverage at a higher cost. However, there are some plans with a national provider network. Also, many retiree plans allow for national coverage. You need to call wife's insurer to determine coverage and costs.

The "Medicare Select" within Mediscare Supplement might have some geographic restrictions.

Ask your doctor.

You may use NV doctor for routine stuff.

For more serious may need St George or another facility,

I have a Medigap plan; the cost is based on where I live, but I've had no problem getting care in another state.

Some plans may not be so flexible, or may not offer coverage some places.
